Despite advances in gastrointestinal endoscopic technology, the management of malignant hilar biliary obstruction continues to be a challenge for advanced endoscopists. There has been ongoing debate regarding optimal modality both diagnostic and therapeutic interventions obstruction. Various approaches including endoscopy, interventional radiology guided surgery are available. The should include multidisciplinary approach, planned based on whether patient is candidate curative surgical resection or not. Understanding Bismuth-Corlette classification as well liver segmental anatomy crucial planning drainage. Safe effective drainage requires detailed understanding ductal anatomy, careful adaptation any strategy individual patient's condition, prognosis, collaboration with surgery, oncology, radiology.
